Pilonidal Sinus Guide

What is a Pilonidal Sinus?

A pilonidal sinus is a small pocket or tunnel in the skin at the top of the buttock crease, just above the tailbone. It often contains hair and skin debris. When it gets infected, it can turn into a painful, swollen lump filled with pus (an abscess).

Some people have only a small pit with no symptoms, while others have repeated flare-ups with pain and discharge. Pilonidal sinus surgery aims to drain the infection and/or remove the sinus. The right approach depends on your condition and is decided by your surgeon after an examination.

 Common signs to watch for

Pain or Swelling Near the Tailbone Pain When Sitting Pus or Blood-Stained Discharge Small Pits or Openings in the Skin Fever with a Painful Swelling

At a Glance

Where it occurs

In the skin at the top of the buttock crease, just above the tailbone (sacrococcygeal region).

Who it commonly affects

More common in young adults and men, people with thick or coarse body hair, and those who sit for long hours.

Why it happens

Loose hairs and skin debris are thought to push into the skin, causing a reaction that forms a pocket or tunnel which can get infected.

How it is diagnosed

Usually by a simple physical examination. Scans are rarely needed, but your surgeon may advise them in complex or repeated cases.

Worth Knowing

Why You Should Not Ignore a Pilonidal Sinus

A pilonidal sinus usually does not heal on its own. Here is what can happen when it is left untreated for a long time.

Repeated Infections

An untreated sinus can flare up again and again, forming painful pus-filled swellings (abscesses).

Tracts Can Spread

Over time the tunnel under the skin may branch out, which can make treatment more extensive.

Daily Life Disruption

Pain while sitting, driving or working at a desk, and frequent dressings, can affect work and routine.

Spreading Infection

Occasionally, infection can spread to the surrounding skin (cellulitis) and may need urgent care.

An evaluation by a surgeon can tell you what your condition needs and what to do next.

Stages of Pilonidal Sinus

Pilonidal sinus can look different from person to person. These are the common ways it shows up.

Early Stage

Pilonidal Cyst Without Infection

A small pit or dimple near the tailbone that may cause little or no pain. Some people only notice it during a routine examination.

📍 Top of the buttock crease⚠️ Get it checked by a surgeon🩺 Surgeon may advise monitoring and hygiene care
Consult now
Needs Prompt Care

Acute Abscess (Infected Swelling)

A sudden, painful, red and warm swelling near the tailbone, sometimes with fever. The pus usually needs to be drained.

📍 Near the tailbone⚠️ See a doctor promptly🩺 Drainage first, surgery may be planned later
Consult now
Long-Standing

Chronic or Recurring Sinus

One or more openings that keep discharging fluid or pus, or a problem that returns after earlier treatment.

📍 Buttock crease / tailbone area⚠️ Plan a surgical consultation🩺 Surgeon will discuss removal of the sinus

Treatment Options

Depending on your condition, your surgeon may recommend one of the following. The right choice depends on whether there is active infection, the size of the sinus and whether it has come back before.

For an Infected Swelling

Drainage of Abscess

When the area is acutely infected, the surgeon may make a small cut to let the pus out (incision and drainage). This relieves pain quickly but does not remove the sinus itself.

Relieves pain and pressure from the pus
Usually a short procedure
Wound dressings needed for some days
Definitive surgery may be advised later
Removing the Sinus

Excision of the Sinus

The sinus tract and the affected tissue are removed. Depending on your case, the wound may be left open to heal gradually from the inside, or closed with stitches.

Removes the tunnel(s) and the infected tissue
Open healing needs regular dressings
Closed wounds need care to stay clean and dry
Choice depends on the size and spread of the sinus
For Larger or Recurring Cases

Excision with Flap Closure

After removing the sinus, the surgeon may move nearby healthy skin (a flap) to close the wound so the scar sits away from the centre of the crease.

Considered for larger or recurring sinuses
Aims to reduce friction on the scar line
Usually done under anaesthesia
Your surgeon will explain if this suits you

Important - Please Read

A pilonidal sinus usually does not go away by itself. Antibiotics may help with infection for a while, but the pocket or tunnel under the skin often remains and can flare up again. A surgeon can examine you and advise whether monitoring, drainage or surgery is right for you.

Recovery varies from person to person. It depends on the size of the sinus, the type of procedure and how the wound is managed (left open or closed). Your surgeon will explain what to expect in your case.

Yes, it can recur in some people. Following the wound-care advice, keeping the area clean and dry, and managing hair in the area as advised by your surgeon can help lower the chance of it coming back.

See a doctor if you notice pain, swelling, redness or discharge near the tailbone, especially with fever. A painful, swollen abscess needs prompt medical attention.
