Thyroid Surgery Guide

What is Thyroid Surgery (Thyroidectomy)?

The thyroid is a butterfly-shaped gland at the front of the neck that makes hormones controlling the body's metabolism. Thyroid surgery (thyroidectomy) is an operation to remove all of the gland (total thyroidectomy) or part of it (partial thyroidectomy or hemithyroidectomy).

Surgery may be advised for a large thyroid swelling (goitre) that presses on the windpipe or food pipe, thyroid lumps (nodules) that are suspicious or confirmed to be cancer, or an overactive thyroid (hyperthyroidism) in selected cases.

Not every thyroid swelling needs surgery. Many nodules are harmless and can be monitored. Your doctor will decide based on your examination, blood tests, ultrasound and, if needed, a needle test (FNAC).

Signs to get your thyroid checked

Swelling or Lump in the Front of the Neck Difficulty Swallowing Breathing Difficulty or Pressure in the Neck Hoarse Voice That Does Not Improve Lump Found on a Scan

At a Glance

Where the thyroid is

A butterfly-shaped gland at the front of the neck, below the Adam's apple.

Tests usually done

Thyroid blood tests, a neck ultrasound and, when needed, a fine needle test (FNAC) of a nodule.

Types of thyroidectomy

Removal of one half of the gland (hemithyroidectomy) or the whole gland (total thyroidectomy).

After surgery

After total thyroidectomy, daily thyroid hormone tablets are needed. After partial removal, your doctor will check if you need them.

Worth Knowing

Why Thyroid Swellings Need Timely Evaluation

Many thyroid swellings are harmless, but some need treatment. Here is why a timely check matters.

Pressure on the Windpipe

A large goitre can press on the windpipe and cause breathing difficulty, especially when lying down.

Difficulty Swallowing

A growing thyroid swelling can press on the food pipe and make swallowing uncomfortable.

Ruling Out Cancer

Most thyroid nodules are not cancer, but tests are needed to find the ones that are, so they can be treated in time.

Hormone Imbalance

An overactive thyroid that is not controlled can affect the heart, weight, energy and mood.

An evaluation by a surgeon can tell you what your condition needs and what to do next.

Common Reasons for Thyroid Surgery

Thyroid surgery is advised for different reasons. These are the most common ones.

Common

Thyroid Swelling (Goitre)

An enlarged thyroid gland. Surgery may be considered if it is large, keeps growing, causes pressure symptoms or for cosmetic concerns.

📍 Front of the neck⚠️ Get an evaluation🩺 Surgery only if your doctor advises it
Consult now
Needs Tests

Thyroid Lump (Nodule)

A lump within the thyroid, often found on a scan. Most are harmless; an ultrasound and needle test (FNAC) help decide what is needed.

📍 Within the thyroid gland⚠️ Ultrasound and FNAC as advised🩺 Many nodules are only monitored

Needs Specialist Care

Thyroid Cancer

When a nodule is suspicious or confirmed to be cancer, surgery to remove part or all of the thyroid is a main part of treatment.

Treatment Options

Depending on your condition and test results, your doctor may recommend one of the following. Your surgeon will explain which option suits you.

When Surgery Is Not Needed

Monitoring & Medicines

Many thyroid nodules and goitres are harmless and are monitored with periodic check-ups and ultrasound. Thyroid hormone problems are often treated with medicines.

Regular follow-up as advised by your doctor
Repeat ultrasound or blood tests when needed
Surgery considered if things change
Partial Removal

Hemithyroidectomy (Lobectomy)

Removal of one half (lobe) of the thyroid gland. It may be advised for a nodule limited to one side or when the diagnosis needs to be confirmed.

One side of the gland is left in place
Removed tissue is sent for lab examination
Your doctor checks if hormone tablets are needed
Complete Removal

Total Thyroidectomy

Removal of the whole thyroid gland. It may be advised for a large goitre, nodules on both sides, some thyroid cancers or certain cases of overactive thyroid.

Lifelong daily thyroid hormone tablets are needed
Calcium levels are checked after surgery
Done under general anaesthesia
For Some Thyroid Cancers

Lymph Node Removal (Neck Dissection)

In some thyroid cancers, nearby lymph nodes in the neck may also be removed along with the thyroid. Your doctors will explain if this applies to you.

Depends on the type and spread of the cancer
Planned after scans and test results
Further treatment, if any, is discussed with you

This information is general. The right treatment for you can only be decided by your surgeon after examining you.

Frequently Asked Questions

General answers to common questions. Your surgeon will advise on your specific condition.

Important - Please Read

No. Many thyroid nodules and goitres are harmless and only need monitoring. Surgery is usually advised when a swelling causes pressure symptoms, keeps growing, is suspicious or confirmed to be cancer, or in some cases of overactive thyroid. Your doctor will advise you based on your tests.

Total thyroidectomy removes the whole thyroid gland. Partial thyroidectomy (hemithyroidectomy or lobectomy) removes one half. The choice depends on the cause, the size and location of the nodules and the test results.

The cost depends on the type of surgery, the reason for it, the tests needed, the length of stay and your insurance cover. Share your details and our team will call you back within 30 minutes with a free cost estimate.

Coverage depends on your policy and whether the procedure is medically necessary - our team checks this for you. Vasavi Hospitals accepts all insurance and provides cashless admission guidance where applicable.

After total thyroidectomy, you will need a daily thyroid hormone tablet for life. After partial removal, many people do not need tablets, but your doctor will check your hormone levels and advise you.

The nerves that control the voice box lie close to the thyroid. Some people have a hoarse or weak voice for a while after surgery; long-term voice changes are less common. Your surgeon will explain the risks, including those related to voice and calcium levels, before surgery.

This varies with the type of surgery and your recovery. Your surgeon will tell you what to expect in your case, including when you can return to work and normal activities.
